Why he might be annoying:

Most of his vocals consist of unintelligible growls and screams rather than any actual singing.
Many of his songs deal with intentionally offensive subject matter such as racism, misogyny, and homophobia.
Anal Cunt were once chased out of Munice, Indiana after he allegedly hit a pregnant woman with a folding chair during a concert.
After years of writing song titles such as 'You Look Divorced' and 'Ha Ha, Your Wife Left You,' he divorced in the early 2000s.
Despite his admitted hatred of heavy metal music, he regularly attends metal concerts for the purpose of moshing and starting fights.
He has numerous feuds with other metal vocalists such as Chris Barnes of Six Feet Under and Jamey Jasta of Hatebreed.
In October 2004, he fell into a near-fatal coma after overdosing on heroin and an entire bottle of sleeping pills.
He claims that his first post-coma concert in May 2005 was the first time in 11 years that he had done a show while sober.

Why he might not be annoying:

His band, Anal Cunt, is often cited as being important and influential in the metal and grindcore subcultures.
He is bold enough to make inflammatory statements in the middle of concerts, even when threatened by angry audience members.
He is not afraid to make fun of himself (including about his near-death experience).
His feud with Chris Barnes is the result of an incident where Barnes challenged him to a fight and then sent a group of roadies to ambush him, rather than fighting him personally.
He has since challenged Barnes to a one-on-one fight but Barnes, as of 2005, has still refused to respond to this offer.
He is in touch with his fans and takes the time to personally respond to fan mail.
He is good friends with Pantera vocalist Phil Anselmo and both have guest-voiced on each other's albums.
